Job summary

The Pennant Group in Morristown, TN is hiring a Social Worker & Bereavement Coordinator for Home Health / Hospice. You will provide psychosocial support to patients and families, coordinate the hospice bereavement program, and work with the interdisciplinary team to plan care transitions and grief counseling.

Requires CSWE-accredited MSW, state licensure, at least 1 year in healthcare/hospice, CPR, and valid driver’s license.

Qualifications

• Master's Degree in Social Work from CSWE-accredited program.
• Current social work licensure as required by state regulations.
• Minimum one year of social work experience in healthcare/home health/hospice; hospice experience preferred.
• Current CPR certification, if required by state regulations or policy.
• Valid driver's license and reliable transportation for work-related travel.

Responsibilities

• Conduct psychosocial assessments of home health patients and identify factors affecting care.
• Develop and implement individualized social work plans with patients/families/caregivers and the interdisciplinary team.
• Provide counseling and support related to illness, adjustment and coping.
• Coordinate referrals to community resources and assist with access to services.
• Maintain timely and accurate clinical documentation per agency/state/federal requirements.
• Coordinate and oversee hospice bereavement program and related services.
• Participate in interdisciplinary group meetings and discharge planning.
