Qualifications

• Master's degree in Social Work from a CSWE-accredited program.
• Current driver's license and access to a dependable vehicle or reliable public transport.
• Preferred CPR certification or ability to obtain within 90 days of hire.

• Assists patients and families to understand social and emotional factors affecting health.
• Contributes to development of a comprehensive Plan of Care with the Interdisciplinary Team.
• Instructs team members on community resources available to support patients.
