Job summary

Reserve Care is hiring a Social Worker to provide counseling and medical social work to terminally ill patients and families in collaboration with the transdisciplinary team in various settings.

You will participate in care conferences, bereavement program, discharge planning, and coordination with volunteer staff, with flexibility for evenings and weekends. Ohio license and MSW required, with 2+ years in counseling preferred.

Qualifications

• Masters prepared Social Worker.
• Licensed in the State of Ohio.
• Experience (2 yrs.) in counseling or related work with individuals, families and groups.

Responsibilities

• Provide counseling and social work to terminally ill patients and families in collaboration with the care team.
• Assist with group counseling and bereavement programs.
• Maintain records and participate in care conferences and on-call rotations.
• Assist with discharge planning and referrals to community resources.
• Coordinate with volunteers and supervise social work interns as assigned.
