Job summary

CorsoCare Hospice is seeking a PRN Social Worker to deliver psychosocial assessments and ongoing counseling for patients and families. This role supports the Hospice IDG and coordinates services across community resources.

Responsibilities include bereavement support, crisis intervention, and ensuring documentation aligns with policy, with flexible scheduling and travel between service areas.

Qualifications

• Must have MSW from CSWE-accredited program or a BSW with supervised MSW experience.
• Ability to provide psychosocial assessments, counseling, and bereavement services.
• Experience in health care settings preferred; strong communication and crisis management skills.

Responsibilities

• Deliver psychosocial, emotional, spiritual assessments and bereavement services.
• Provide ongoing counseling and crisis intervention as part of the care plan.
• Collaborate with IDG and community resources to coordinate referrals.
