Job summary

TriHealth Hospice in Cincinnati is seeking a Social Worker to support patients and families in long-term care settings. You will assess psychosocial needs, develop comprehensive care plans, and collaborate with the multidisciplinary team to ensure smooth transitions across care levels.

The role is Monday–Friday days, with on-call coverage including some weekends and holidays. A MSW, Ohio licensure, and up to 1 year of clinical healthcare or community social work are required.

Qualifications

• Master's Degree in Social Science or Social Work is required.
• Up to 1 year of clinical healthcare or community social work experience is required.
• Ohio Licensed Social Worker is required.

Responsibilities

• Applies root cause analysis to readmissions and works with the health care team, patient, and family to resolve issues.
• Communicates with the multidisciplinary team to advise the plan and patient response to the plan.
• Develops and implements plan of care based on assessment including counseling or crisis intervention, and facilitates linkages to community resources.
• Promotes the profession of social work by providing education and teaching rounds; serves as a resource for new staff.
• Responds to referrals and assesses psychosocial and complex discharge needs of patients and families.
