Job summary

Buckeye Hospice and Palliative Care Services is seeking a Medical Social Worker to provide psychosocial support to patients and families within the hospice program. The role includes psychosocial assessments, participation in IDG meetings, and ongoing care coordination.

The ideal candidate will hold an OH licensed social worker credential (MSW CSWE-accredited or BSW with MSW supervision). A valid driver’s license, hospice experience, and strong written and verbal communication are preferred.

Qualifications

• Must be licensed to practice social work in Ohio.
• MSW from CSWE-accredited program or BSW with MSW supervision option.
• Hospice experience preferred.
• Must have a valid driver's license.
• Ability to communicate well, both verbally and in writing.

Responsibilities

• Provide Medical Social Services to patients and families as per Plan of Care.
• Conduct psychosocial assessments within 5 days of admission.
• Participate in IDG meetings and develop patient plans of care.
• Record and report changes in emotional/social status.
• Submit initial evaluation, care plan, and daily notes.
