Job summary

Reserve Care in Ohio is seeking a Licensed Master Social Worker to join our hospice and palliative care team. You will provide counseling, coordinate with the transdisciplinary team, and support patients, families, and caregivers in various settings.

Responsibilities include psychosocial assessment, discharge planning, bereavement support, case management, and contributing to quality improvement while working flexible hours and an on-call rotation.

Qualifications

• Masters prepared Social Worker.
• Licensed in the State of Ohio.
• 2 yrs. experience in counseling or related work.
• Experience preferred in hospice, medical facilities or nursing homes.
• Knowledge of community resources.
• Must have and maintain a valid driver’s license.

Responsibilities

• Address emotional, spiritual and psychosocial needs of patients and families.
• Assist with group counseling and support programs.
• Maintain and complete records and reports per agency standards.
• Participate in care conferences and bereavement program.
• Assist with discharge planning and referrals to resources.
• Coordinate with volunteer staff and participate in case reviews.
• Supervise Social Work Interns as assigned.
