Job summary

Oasis Hospice is seeking a dedicated Social Worker to deliver psychosocial, emotional, and spiritual support to hospice patients and families. You will conduct assessments, provide counseling, bereavement services, and community outreach as part of the interdisciplinary group.

Must meet CSWE-accredited MSW options or equivalent supervision pathways. Minimum two years of experience preferred; strong independence, crisis response, and interpersonal skills are essential.

Qualifications

• MSW degree from CSWE with 1 year health care experience.
• BSW with MSW supervision requirement and experience in health care.
• BA in psychology, sociology or related field with 1 year health care experience and MSW supervision.

Responsibilities

• Perform initial psychosocial, emotional, spiritual and bereavement assessments.
• Develop and implement goal-directed IDG care plans with the physician.
• Provide ongoing counseling and crisis intervention as needed.
• Deliver services to hospice patients and families under IDG framework.
• Maintain thorough documentation of psychosocial assessments and interventions.
• Collaborate with community resources to support patients and families.
