Job summary

Bluesummithospice is seeking a Hospice Social Worker to deliver psychosocial, emotional, spiritual and bereavement services to patients and families. The role includes initial assessments, ongoing counseling, and community education as part of the Hospice IDG.

Requirements include an MSW from a CSWE-accredited program with one year in a health-care setting or a BSW with supervision by an MSW, or a related BA with supervision and state-compliant credentials.

Qualifications

• MSW degree from CSWE accredited school and one year health-care experience.
• BSW from CSWE accredited school with one year health-care experience supervised by an MSW.
• BA in psychology/sociology with one year health-care experience supervised by an MSW; state requirements may apply.

Responsibilities

• Perform initial psychosocial, emotional, spiritual and bereavement assessments and assist in care plan development.
• Provide ongoing counseling and crisis intervention as required.
• Advocate for patients and families and coordinate with community resources and services.
• Document comprehensive psychosocial, emotional and spiritual assessments in the patient record.
• Attend team meetings and maintain professional development.
