Qualifications

• Licensed Clinical Social Worker (LCSW) in good standing.
• Driver’s License (State issued).
• Proof of Auto Insurance.
• Licensed Social Worker (LSW) in good standing.
• Master’s Degree in Social Work is required; hospice experience preferred.

Responsibilities

• Provide psychosocial assessments of patients and families.
• Develop bereavement plan of care for families.
• Identify and communicate religious/cultural considerations.
• Educate patients and families to overcome barriers to care.
• Document assessments and interventions in EMR and coordinate with IDT.
