Job summary

Four Seasons Compassion for Life is seeking a Social Worker to provide psychosocial services to hospice patients and families. You will assess needs, develop care plans, coordinate with community resources, and support families through bereavement and end-of-life planning.

The role requires managing an assigned caseload, collaborating with IDG teams, and navigating Medicare/participation guidelines while delivering compassionate care in a variety of settings.

Qualifications

• Bachelor’s in Social Work (BSW) or Master’s in Social Work (MSW).
• 1–3 years of experience in a healthcare setting preferred.
• Valid driver’s license and proof of automobile insurance.

Responsibilities

• Provide social work services and psychosocial assessments.
• Develop and evaluate care plans for hospice patients.
• Liaise with community health and social agencies; referrals as needed.
• Counsel patients and families on goals, coping, and bereavement.
• Coordinate discharge planning and IDG team involvement.
• Address financial needs and assist with financial planning.
