Job summary

Reserve Care in Cleveland, OH is seeking a Social Worker to provide counseling and medical social work to terminally ill patients and families, collaborating with the transdisciplinary team in a variety of settings.

You will address emotional, spiritual and psychosocial needs, participate in care conferences and bereavement programs, assist with discharge planning, referrals, charting and on-call rotations, including evenings and weekends.

Qualifications

• Master’s degree in Social Work required.
• Licensed in the State of Ohio.
• 2+ years experience in counseling or related work with individuals, families and groups.
• Hospice/medical facility or nursing home experience preferred.
• Knowledge of community resources.
• Valid driver’s license with good driving record.
• Equivalent education/experience may substitute for minimum qualifications.

Responsibilities

• Provide counseling and medical social work to terminally ill patients and families.
• Assist with group counseling and support programs for patients and families/caregivers.
• Maintain and promptly complete required records and reports.
• Participate in care conferences and bereavement program.
• Assist with discharge planning and referrals to community resources.
• Participate in on-call rotation as scheduled.
• Coordinate and communicate with volunteers and interdisciplinary team.
• Complete annual competency requirements.
