Continuum Care of Florida is seeking an experienced MSW to implement psychosocial care standards for hospice patients and families. The role supports the interdisciplinary group, conducts assessments, and provides crisis intervention aligned with the plan of care.

The MSW will educate on advanced directives, assist in care planning, and coordinate with physicians and community resources to ensure comprehensive, patient-centered care. A valid driver's license and CPR certification are required.

Qualifikationen

• MSW degree from an CSWE-accredited program.
• Hospice experience preferred.
• Licensed driver with insured vehicle.
• CPR certification (in-person if required).

Aufgaben

• Assess psychosocial status of patients and families/caregivers and communicate findings to the team.
• Provide crisis intervention and coping support as needed.
• Educate on advance directives and care planning.
• Participate in development and execution of the plan of care with the interdisciplinary group.
• Assist in understanding social and emotional factors related to health problems and end-of-life decisions.
• Attend interdisciplinary meetings and maintain accurate clinical notes.
