Job summary

Blue Ridge Care is seeking a Hospice Social Worker to address emotional and psychosocial needs of patients and families facing advanced illness. You will collaborate with the care team, provide counseling, complete social evaluations, and coordinate community resources to support safe, dignified home-based care.

The role requires active engagement with the interdisciplinary team, strong documentation, and travel within the service area to deliver personalized care.

Qualifications

• Bachelor of Science in Social Work, Psychology, or Sociology; MSW preferred.
• At least one year of experience in a health-care setting; hospice experience preferred.
• Ability to travel throughout the Blue Ridge Care service area in Loudoun County.

Responsibilities

• Assess psychosocial status and coordinate with the care team to guide planning.
• Assess safety and environmental conditions in the home and adjust plans as needed.
• Complete social evaluations and implement interventions to support coping.
• Provide counseling to patients and families on stress and end-of-life issues.
• Maintain accurate clinical records in the EMR and document bedside assessments.
• Coordinate information and referrals with community resources and agencies.
• Collaborate with interdisciplinary team and participate in IDT meetings.
• Support quality improvement activities related to social work care.
