Job summary

Blue Ridge Care Services is seeking a Medical Social Worker (Hospice) to support patients and families with compassionate, person-centered care. You will join a dedicated interdisciplinary team, guiding end-of-life discussions and coordinating resources across care settings.

Required: bachelor's degree in a related field, 1+ year healthcare experience, excellent communication, and a valid driver's license. MSW preferred and hospice experience encouraged.

Responsibilities

• Help caregivers navigate stress, grief, loss, and end-of-life transitions.
• Connect patients with community resources and support services.
• Collaborate closely with nurses, physicians, chaplains, and other care team members.
• Advocate for patient needs and coordinate services across care settings.
• Maintain accurate documentation and participate in care planning meetings.
• Travel throughout the service area to provide support where patients call home.

What We're Looking For

Required:

Preferred:

• Master of Social Work (MSW)
• Hospice, palliative care, home health, or healthcare social work experience
• Knowledge of hospice philosophy, grief support, and end-of-life care
