Job summary

Blue Ridge Care is seeking a Hospice Social Worker to address emotional and psychosocial needs of patients and families living with advanced illness. You will collaborate with an interdisciplinary team to implement individualized care plans in the home setting.

Responsibilities include counseling, assessments, resource coordination, and maintaining accurate EMR records while guiding families through end-of-life decisions with compassion.

Qualifications

• Bachelor of Science in Social Work, Psychology, or Sociology; MSW preferred.
• At least one year of experience in a health-care setting; hospice experience preferred.
• Understanding of hospice philosophy and issues related to death and dying.
• Strong communication, documentation, and organizational skills.
• Ability to travel throughout the Blue Ridge Care service area in Loudoun County.

Responsibilities

• Assess psychosocial needs of patients and families.
• Provide counseling and develop care plans with the IDT.
• Maintain EMR documentation.
• Coordinate information and referrals.
• Liaison with families and community partners.
• Participate in quality improvement activities.
