Job summary

Blue Ridge Care is seeking a compassionate Registered Nurse (RN) Case Manager to join our hospice team and serve patients and families with dignity and empathy. You will lead personalized care planning and coordinate services across clinicians and support staff.

Qualifications

• Current Virginia RN license (single-state or multistate).
• CPR certification.
• Minimum two years of nursing experience.
• Hospice, palliative care, or home health experience preferred.
• Strong communication, assessment, and organizational skills.
• Valid driver's license, reliable transportation, and automobile insurance.
• A compassionate, self-motivated approach to patient-centered care.

Responsibilities

• Assess patient and family needs and develop individualized care plans.
• Deliver skilled nursing care focused on comfort, symptom management, and quality of life.
• Coordinate care among physicians, hospice aides, social workers, chaplains, and other team members.
• Educate and support families through every stage of the hospice journey.
• Supervise and guide hospice aides to ensure exceptional patient care.
• Maintain accurate clinical documentation and participate in ongoing professional development.
