Job summary

Blue Ridge Care is seeking a compassionate Registered Nurse (RN) to join our Hospice team. The role focuses on planning, organizing, and directing hospice care for patients and families in a community-based setting.

The ideal candidate is clinically skilled, compassionate, and self-directed, capable of managing complex patient needs and providing emotional support during sensitive times.

Qualifications

• Must be licensed or eligible for licensure as a Registered Nurse in Virginia.
• Current CPR certification required.
• Minimum two years of nursing experience; hospice/public health/home care preferred.
• Strong assessment, communication, and critical thinking skills.
• Ability to work independently and manage high-acuity patients.

Responsibilities

• Perform initial and ongoing patient and family assessments.
• Provide professional nursing care using the nursing process.
• Develop and implement individualized hospice care plans.
• Monitor and adjust care plans based on patient needs.
• Administer medications per physician orders.
• Educate patients and caregivers on care plans and self-care.
• Assist with activities of daily living and comfort measures.
• Maintain accurate EMR documentation and reporting.
• Collaborate with interdisciplinary team for coordinated care.
• Participate in on-call rotations and ensure safety.
• Support families emotionally and participate in care planning.
