Job summary

Blue Ridge Care is seeking a Hospice Social Worker to support patients, caregivers, and families by addressing emotional and psychosocial needs in home-based end-of-life care. You will collaborate with the interdisciplinary team to develop and support an individualized plan of care, providing counseling, assessments, and resource coordination.

You will maintain accurate EMR records, assess home safety, connect families with community resources, and participate in IDT meetings to guide care

Qualifications

• BS in Social Work, Psychology, or Sociology; MSW preferred.
• At least 1 year in a health-care setting; hospice experience preferred.
• Understanding of hospice philosophy and end-of-life issues.
• Strong communication, documentation, and organizational skills.
• Ability to travel throughout the Blue Ridge Care service area in Loudoun County.

Responsibilities

• Assess psychosocial needs of patients and caregivers.
• Provide counseling related to stress, illness progression, and end-of-life issues.
• Develop and implement interventions to support stability and coping.
• Maintain accurate clinical records in the EMR.
• Coordinate information and referrals to community resources.
• Liaise between families, hospice team, and community partners.
• Participate in IDT meetings and care planning.
