The Pennant Group, Inc. is seeking a Medical Social Worker to implement standards of care for social work services within hospice programs.

The role involves assessing psychosocial status of patients and families, planning interventions, and collaborating with nurses and the interdisciplinary team. You will maintain patient records, provide information and referrals, and participate in quality improvement activities.

Competenze

• BSW from CSWE-accredited program.
• Valid CPR certification.
• Excellent verbal and written communication.
• Ability to drive and have reliable transportation.
• Hospice care experience preferred.

Mansioni

• Assesses psychosocial status of patients and families; communicates findings to care team.
• Performs home-based social work assessments and plans caregiver support.
• Maintains clinical records for referred patients.
• Provides information and referrals to patients, families, and agencies.
• Participates in interdisciplinary team meetings and quality improvement activities.
