Job summary

Qualifications

Responsibilities

• Conduct psychosocial assessments of patients and families to identify needs and resources.
• Develop bereavement plan of care for families and caregivers.
• Advise on spiritual/cultural considerations impacting care.
• Educate patients and families to overcome barriers and match learning needs.
• Document assessments and interventions in the EMR in a timely manner.
• Collaborate with the IDT to implement the social work plan of care.
• Support patient decisions and end-of-life planning discussions.
• Identify and respond to changes in patient/family situations and adjust plans.
