Job summary

St. Croix Hospice is seeking a Social Worker to support patients and families through the end-of-life journey. You will meet with patients, collaborate with the clinical team, and coordinate access to community resources and services.

The role requires a CSWE-accredited social work degree, current licensure, and the ability to conduct psychosocial assessments, provide counseling, and coordinate care. A compassionate, autonomous practice is valued.

Qualifications

• Bachelor’s degree in social work from an CSWE-accredited program.
• Current Social Work licensed in the state(s) of practice.
• Six licensure levels: LBSW, LMSW, Limited LMSW, LCSW, SST, Limited SST.
• Ability to pass background, consumer report, and drug test.
• Conduct psychosocial assessments, identify safety or support needs, and provide resources, referrals, and care planning support for patients and families.
• Provide counseling, crisis intervention, and bereavement support, including follow up for at risk individuals and coordination of bereavement activities.
• Collaborate with the interdisciplinary team, maintain accurate documentation, support cultural and end of life needs, liaise with community agencies, and ensure HIPAA compliance.
• Must have reliable transportation, valid driver’s license, and proof of auto insurance

Responsibilities

• Meet with patients and their loved ones to provide psychosocial support and resources.
• Collaborate with the clinical team to coordinate care and treatment planning.
• Help families access appropriate community resources and services.
• Conduct psychosocial assessments and support care planning and referrals.
• Provide counseling, crisis intervention, and bereavement support, with follow-up as needed.
• Maintain HIPAA-compliant documentation and coordinate with community agencies.
