Job summary

St. Croix Hospice in Bemidji, MN is seeking a licensed Social Worker to join a dedicated patient-care team and provide compassionate psychosocial support to patients and families during end-of-life care.

You will conduct assessments, help with care planning, coordinate with clinicians, and offer counseling and bereavement support while ensuring HIPAA compliance and access to community resources.

Qualifications

• Bachelor’s degree in social work from CSWE-accredited program.
• Current social work license in the state of practice (Minnesota).
• Ability to pass background, consumer report, and drug test.

Responsibilities

• Conduct psychosocial assessments and identify safety or support needs.
• Provide counseling, crisis intervention, and bereavement support.
• Collaborate with the interdisciplinary team and ensure HIPAA compliance.
• Assist with resources, referrals, and care planning for patients and families.
• Maintain accurate documentation and coordinate with community agencies.
