St. Croix Hospice is seeking a Social Worker to support patients and families through the end-of-life journey. You will meet with patients, collaborate with the clinical team, and coordinate access to community resources and services.
The role requires a CSWE-accredited social work degree, current licensure, and the ability to conduct psychosocial assessments, provide counseling, and coordinate care. A compassionate, autonomous practice is valued.
