Job summary

Qualifications

• Master's degree in Social Work.
• LSW and LCSW licensure required (IDFPR).
• Hospice experience of 1+ years.
• Driver's license and proof of auto insurance.

Responsibilities

• Provide psychosocial assessments of patients and families.
• Develop bereavement plan of care.
• Interpret and respect patient/family faith and culture traditions.
• Educate patients and families to overcome barriers and meet needs.
• Document assessments and interventions in EMR.
• Develop and adjust social work plan with IDT.
• Support patient decisions and end-of-life care preferences.
• Identify indicators of imminent death and address needs at time of death.
