Job summary

UnitedHealth Group is seeking a part-time Social Worker to provide medical social services under physician direction and an Interdisciplinary Group for patients’ social and emotional health needs.

Responsibilities include contributing to an integrated Plan of Care, coordinating with the health care team, and guiding access to community resources. Salary ranges align with full-time equivalents and benefits are offered.

Qualifications

• Master's Degree from a school of Social Work accredited by the Council on Social Work Education (CSWE).
• Current Driver's License, vehicle insurance, and access to dependable transportation.

Responsibilities

• Assists patient, family, physician and health care team to understand personal, emotional and social factors related to health status.
• Contributes to development of a comprehensive, integrated Plan of Care for patients daily.
• Instructs team members on community resources available to assist patients as needed.
