Job summary

VITAS Healthcare in Illinois is seeking a hospice social worker to provide emotional and logistical support to patients and families at the end of life. You will help ensure affairs are in order and support loved ones through grieving, with regular collaboration with an interdisciplinary team.

Qualifications

• MSW from a CSWE-accredited program.
• At least 1 year of social work experience in health care.
• Experience with terminally ill patients and families preferred.
• Licensure/certification as required by state regulations.

Responsibilities

• Provide emotional and logistical support to patients and families.
• Help patients' affairs be in order (finances, funeral plans, etc.).
• Support and guide families through grief and end-of-life planning.
• Visit patients at home; collaborate with interdisciplinary team.
• Educate patients and families on emotional responses to illness progression.
