Job summary

VITAS Healthcare, the nation's leading end-of-life care provider, is seeking a hospice social worker. You will provide emotional and logistical support to patients and their families, helping ensure end-of-life wishes, finances, and funeral arrangements are handled with dignity.

You will work in patients' homes or common care settings, partnering with an interdisciplinary team to deliver compassionate, patient- and family-centered care across diverse communities.

Qualifications

• MSW required from CSWE-accredited school.
• At least one year of social work experience in healthcare.
• Experience with terminally ill patients and families preferred.
• Counseling and support skills essential.
• License or certification as required by state.

Responsibilities

• Provide emotional and logistical support to patients and families.
• Help arrange finances, funeral arrangements, and end-of-life wishes.
• Assist families navigating the grieving process.
• Collaborate with RNs, physicians, chaplains and the Interdisciplinary Team.
• Travel to patient homes as required.
