Job summary

Compassus is seeking a Social Worker and Bereavement Coordinator to support patients and families with psychosocial care, bereavement services, and care planning. You will collaborate with the interdisciplinary team to implement plans of care and coordinate community resources.

The role requires a Bachelor’s degree in a relevant field, with pediatric or adult hospice experience preferred; licensure as applicable by state rules.

Qualifications

• Bachelor’s degree in Social Work, Psychology, Sociology, or related field with 1 year of healthcare social work experience.
• Preferred: Master’s degree in Social Work with 1 year of healthcare social work experience.
• Minimum 2 years of grief, loss or bereavement support experience.

Responsibilities

• Provide psycho-social and bereavement support to patients and families; engage community resources as needed.
• Conduct initial and ongoing psychosocial assessments; address financial resources, living arrangements, and caregiver stressors.
• Promote advance care planning and assist with living wills and POA documentation.
• Supervise social work practicum students when applicable.
• Collaborate with IDT to develop and implement plans of care; ensure timely documentation.
