Job summary

Compassus in Princeton, WV is seeking a Social Worker and Bereavement Coordinator to provide psycho-social and bereavement support within the hospice team. You will participate in IDT care planning, develop care plans, and help families navigate resources and advance care planning while upholding Compassus values.

Required bachelor’s in social work or related field with 1 year of healthcare social work; licensure as needed by state rules.

Qualifications

• Bachelor's degree in Social Work, Psychology, Sociology, or related field; 1 year healthcare social work experience; license if state rules require.
• Master's degree in Social Work with 1 year healthcare social work experience preferred; licensure as required.
• Minimum 2 years bereavement or grief support experience with individuals and families.

Responsibilities

• Explain hospice services and Medicare benefits; obtain required consents and documentation.
• Provide psycho-social and bereavement support to patients, families, and caregivers.
• Develop, update, and maintain care plans reflecting current problems and goals.
• Participate as a member of the interdisciplinary team (IDT) in care planning.
• Conduct psycho-social assessments addressing finances, insurance, and community needs.
• Facilitate long-term care placement and participate in care planning meetings.
• Promote bereavement risk factors and implement bereavement care plans after death.
• Supervise social work practicum students as applicable.
