Job summary

Compassus is seeking a Hospice Social Worker to provide psychosocial care to patients and families, coordinating with the interdisciplinary team to implement care plans and promote quality of life. The role emphasizes compassionate counseling, informed consent, and timely documentation within a hospice setting.

Qualified candidates hold an MSW from a CSWE-accredited program with healthcare experience and must have LMSW/LCSW/LICSW licensure as required by the state.

Qualifications

• MSW from CSWE-accredited program with at least 1 year healthcare experience.
• Bachelor’s degree in social work or related field with 1 year healthcare experience may be considered where MSW is not available.
• Licensure: LMSW, LCSW, or LICSW; state licensure as required.

Responsibilities

• Explain hospice services and Medicare benefits to patients and families; obtain informed consent and election of benefits documents.
• Submit documentation to facilities promptly after patient visits; ensure policy-compliant reporting.
• Participate in IDT and contribute to plan of care.
• Provide psycho-social support and counseling to patient and family; refer to external resources as needed.
