Job summary

Samaritan Healthcare & Hospice in Mount Laurel, NJ is seeking a licensed social worker to provide psychosocial and bereavement support to hospice patients and their families as part of the interdisciplinary team.

You will assess needs, develop care plans, deliver counseling, connect families with community resources, and provide crisis intervention while collaborating with clinicians and staff.

Qualifications

• Masters of Social Work from an accredited school; LSW or LCSW; LCSW preferred.
• Must have knowledge of community resources.
• Valid Driver’s License and evidence of auto insurance.

Responsibilities

• Assess needs of patients and families and develop a plan of care with the health care team.
• Provide emotional support, counseling and education to patients and families.
• Facilitate participation in treatment activities and coordinate community resources.
• Provide crisis intervention and supportive services to patients, families and staff.
• Follow up with patients and families based on level of care and plan of care.
• Collaborate with the interdisciplinary care team to ensure quality services.
• Support bereavement care and continuity of care planning after patient loss.
