Job summary

MJHS is a large not-for-profit health system in the Greater New York area seeking a Social Worker to provide psychosocial support to Hospice patients and their families across inpatient, home care and nursing home settings, including bereavement.

The role involves collaborating with an interdisciplinary team, developing social work plans, and coordinating discharge plans. NYS MSW licensure and a valid NYS driver’s license are required.

Qualifications

• Minimum of one year experience in Social Work in a healthcare setting.
• Knowledge of Word and Excel preferred.
• Ability to work within an interdisciplinary team environment.
• Ability to input clinical data proficiently.

Responsibilities

• Provide psychosocial support and other Social Work services to hospice patients and their families in inpatient, home care/nursing home settings, and during pre/post bereavement phases.
• Serve as social work member of hospice IDG and as liaison between hospice staff and inpatient/nursing home staff.
• Develop social work plan of care, goals and recommendations based on patient/family needs for assigned patients admitted to inpatient units or nursing home/hospice residents.
• Develop social work plan of care and discharge plans when appropriate for patients admitted into inpatient facility.
