Qualifications

• Master of Social Work degree required.
• Hospice experience preferred.
• Valid driver's license and ability to travel across the service area.
• Strong psychosocial assessment and counseling skills.

Responsibilities

• Perform psychosocial, emotional, spiritual and bereavement assessments.
• Develop and implement goal-directed care plans with IDG involvement.
• Provide ongoing counseling to patients and families.
• Coordinate with physicians and Bereavement Team as assigned.
• Document assessments and contacts in the patient record per policy.
• Engage with community agencies to promote referrals and support.
