Job summary

Samaritan, a not-for-profit hospice provider, seeks a Social Worker to deliver psychosocial and bereavement support to patients and families while collaborating with the hospice Interdisciplinary Care Team.

You will assess needs, develop care plans, provide counseling, coordinate community resources, and assist with difficult decisions. Must have MSW and licensure; driver’s license required; experience in medical settings preferred.

Qualifications

• Masters of Social Work from an accredited school of social work or related field.
• LSW or LCSW; LCSW preferred.
• Valid Driver’s License.
• Knowledge of community resources.

Responsibilities

• Assess psychosocial and bereavement needs of patients and families and develop a plan of care with the health care team.
• Facilitate patient and family participation in treatment activities.
• Provide emotional support, counseling and education to patients and families to ease stress and support decision making.
• Implement intervention strategies and referrals to community resources to ensure continuity of care.
• Provide crisis intervention and supportive services to patients, families, staff and community partners.
• Provide timely feedback and follow-up to patients, families, and the care team.
