Job summary

Suncoast Hospice of Hillsborough, a part of Empath Health, is seeking a Social Worker to support patients and families during their end-of-life journey. The role provides psychosocial support, care coordination, and resource assistance to ensure comfort and dignity for those we serve in North Hillsborough County.

Minimum one year of social work experience in a healthcare setting is required, with Florida licensure or eligibility within 90 days of hire.

Responsibilities

• Assess patient and family needs related to coping, mental health, financial resources, and caregiver support.

• Collaborate with the Interdisciplinary Group (IDG) to develop and manage patient care plans.
• Assist with financial resources, Medicare/Medicaid, and long-term care planning.
• Maintain accurate EMR and participate in IDG meetings.
