Job summary

Suncoast Hospice of Hillsborough, part of Empath Health, seeks an RN Case Manager to provide in-home and facility-based hospice care with daytime hours, Monday–Friday.

You will assess patients, develop Plans of Care with the IDT, supervise LPNs and aides, and guide families through the hospice journey with compassion and dignity.

A Florida RN license and driver’s license are required; bilingual Spanish/English preferred.

Qualifications

• Associate degree or equivalent required.
• Valid Florida RN license required.
• Valid Florida driver’s license required.
• One year related experience preferred.

Responsibilities

• Obtain patient history and perform age-appropriate assessments; initiate referrals and collaborate with IDT and providers.
• Develop and update the Plan of Care with patient, family, IDT, and care partners; monitor progress toward goals.
• Provide continuity of care and care coordination, including post-death closure support with IDT.
• Supervise and direct LPNs and Home Health Aides; monitor service delivery and conduct supervisor visits.

Suncoast Hospice of Hillsborough, a member of Empath Health, is seeking an RN Case Manager to join the Empath team. Monday - Friday, daytime hours. Coverage area includes North Hillsborough County, providing direct patient care in private homes and facilities. As an RN Case Manager, you'll build meaningful relationships with patients and families, guiding them through their hospice journey with compassion, expertise, and dignity.

What You’ll Do

• Demonstrates the ability to obtain patient history, perform an age-specific physical, psychosocial and spiritual assessment and initiates appropriate referral, intervention and collaboration between the patient, family, Interdisciplinary team (IDT), physician and partnering facility (as applicable) following policy and practice guidelines.
• As RN Case Manager provides continuity of care, care coordination, identifies patient/family goals, develops a Plan of Care with patient, family, IDT, attending and physician, partnering facility and evaluates progress towards goals. The Plan of Care will be updated according to policy/guidelines and as needs change.
• Following a patient’s death, assists with closure contact and support in collaboration with IDT.
• Directs and supervises the care provided by the LPN and Home Health Aide following agency policy. Evaluates services provided by the Home Health Aide every two weeks with periodic on-site supervisory visits.
• Demonstrates the ability to administer, monitor and document medications as prescribed by attending physician. Uses age-specific care practices and provide appropriate patient/family teaching.

What You’ll Need

• Education and/or Experience: Associate’s degree (A.A.) or equivalent from two-year college or technical school; Minimum of one year related experience and/or training; or equivalent combination of education and experience.
• Valid Florida license as a Registered Nurse
• Valid Florida State Driver’s License
• Prefer one (1) year experience in an acute or post acute care setting
• Bilingual Spanish/English preferred
• For field-based positions, employees must have reliable transportation which will enable them to perform tasks and responsibilities in a timely and appropriate fashion. Must provide proof of valid automobile insurance, a copy of which will be placed in the employee’s HR file.
