Job summary

Residential Home Health and Hospice is seeking a Hospice RN to deliver compassionate care on weekends within Brighton, MI. You will work with an interdisciplinary team to uphold comfort, dignity, and quality of life for seriously ill patients.

Expect to provide symptom management, support families, and maintain accurate EMR documentation while traveling to patient homes across the assigned territory. The role offers autonomy and meaningful patient relationships.

Qualifications

• Active Registered Nurse license in the state you work.
• Two years of RN experience with hospice care preferred.
• Valid driver’s license and ability to spend ~20% of time driving.

Responsibilities

• Deliver compassionate, patient-centered hospice care with an interdisciplinary team.
• Respond to urgent needs, manage symptoms and pain, provide end-of-life support.
• Support families during weekends, keeping them informed and reassured.
• Coordinate care with strong clinical judgment and timely EMR documentation.
• Work independently, but collaborate with teammates as needed.
• Travel to patient homes within assigned territory.

What You’ll Do

• Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life


• Respond to patient needs by performing urgent assessments, managing symptoms and pain, and providing compassionate end-of-life support in real time


• Support and guide patients and families during weekends, and crisis situations, ensuring they feel informed, reassured, and cared for throughout the hospice journey


• Apply strong clinical judgment and critical thinking to coordinate care needs, respond to changing patient conditions, and maintain accurate, timely, and compliant EMR documentation


• Work independently, but never alone


• Travel to patient homes within assigned territory

Weekend RN Qualifications

• Active Registered Nurse, RN license in the state you work


• Two years of RN experience and the clinical expertise to deliver quality patient care, Hospice experience preferred


• Due to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required
