Qualifications
- Associate degree from an accredited School of Nursing.
- Minimum of two (2) years of experience as a Registered Nurse (RN).
- Experience with EPIC and HCHB preferred.
- Proficiency with computers and general office software.
Responsibilities
- Conduct initial and ongoing patient/family assessments to determine hospice needs.
- Coordinate start-of-care and ensure proper documentation and care planning.
- Educate patients and families on hospice services and disease progression.
