Job summary

Graham Healthcare Group is seeking a Registered Nurse (RN) for Hospice to provide compassionate end-of-life care to patients in their homes in Prospect Heights, IL. This is a staff position with 40 hours per week and 8-hour shifts.

Travel to patient locations within the territory is required. A $10,000 first-year success bonus is offered to reward milestone achievements during the first year.

Qualifications

• Active RN license in Illinois.
• Two years of RN experience; hospice experience preferred.
• Valid driver's license and ability to travel ~20% of workday.

Responsibilities

• Deliver patient-centered hospice care in patients' homes.
• Perform comprehensive assessments, symptom and pain management.
• Educate and support patients and families through end-of-life care.
• Maintain thorough EMR documentation and compliance.
• Travel to patient homes within assigned territory.
