Job summary

AccentCare, Inc. is seeking a Hospice Registered Nurse Team Director in Norwood, MA. This full-time role offers a $105,000–$120,000 annual salary with a $10,000 sign-on bonus and a Monday–Friday schedule including administrative on-call duties.

TheRN will supervise the care team and manage scheduling, documentation, and interdisciplinary care planning to ensure high-quality patient outcomes. Qualified candidates have an Associate degree (CHPN preferred), 3+ years in hospice/home health, and at

Qualifications

• Associate degree preferred.
• CHPN certification preferred.
• Three+ years in hospice, home health, or related nursing.
• Minimum one year of supervisory experience.
• CPR certification preferred.
• Licensed driver with ability to travel to all locations.
• Meet state regulatory requirements where the program provides services.

Responsibilities

• Supervise patient and family care team members with site leadership.
• Handle scheduling, payroll and administrative duties.
• Ensure initial and reassessment documentation within required timeframes.
• Oversee interdisciplinary group care planning for each patient/family.
• Mentor staff in patient care activities and documentation.
• Support staff education efforts and IDG meetings.
• Provide IDG triage and quality calls and support.
• Contribute to quality improvement activities and communications.
