Job summary

AccentCare is seeking a full-time Hospice RN Team Director in Norwood, MA. Salary range is $105,000–$120,000 per year with a $10,000 sign-on bonus and a Monday–Friday schedule with administrative on-call duties.

The role requires supervisory experience and a mix of clinical leadership across interdisciplinary teams to coordinate patient care in hospice services.

Qualifications

• Associate degree preferred
• Certified Hospice Nurse (CHPN) preferred
• Three+ years' experience in hospice, home health, medical/surgical, oncology, or geriatrics/pediatrics
• Minimum of one year of supervisory experience
• Must have CPR certification (preferred)
• Must be a licensed driver who can travel to all locations
• Meet regulations of state(s) where services are provided

Responsibilities

• Supervise direct patient and family care team members with site leadership
• Manage scheduling, payroll and other administrative duties
• Ensure initial assessments and required reassessments are completed timely
• Oversee IDG development, implementation and review of care plans
• Collaborate with site leadership to mentor staff and monitor productivity
• Support Back to Bedside and case management skills per protocol
• Support staff education efforts with site leadership
• Facilitate IDG meetings and provide triage and caller support
• Participate in quality improvement activities and data collection
• Reinforce internal and external communications

What You Need to Know

Be the Best Hospice Registered Nurse/RN Team Director You Can Be

• Associate degree preferred
• Certified Hospice Nurse (CHPN) preferred
• Three+ years' experience in hospice, home health, general medical surgery, oncology, or patient population-specific nursing (i.e., geriatrics, pediatric, etc.)
• Minimum of one year of supervisory experience
• Possess and maintain valid CPR certification while employed in a clinical role (preferred)
• Must be a licensed driver who can travel to all business locations
• Meet the regulations and requirements of the state(s) in which the program provides services

Responsibilities

• Supervise direct patient and family care team members, in collaboration with program site leadership
• Responsible for scheduling, payroll, and other administrative duties
• Ensure all initial assessments and required reassessments are conducted by qualified team members within required time frames, including documentation submissions, and/or synchronization
• Oversee and support the Interdisciplinary Group (IDG) in development, implementation, and review of the interdicisiplinary plan of care for each patient and family
• Collaborate with program site leadership memebrs to monitor, manage, and mentor staff in patient care activities, productivity, and documentation per related policies and protocols
• Pariticpates and promotes Back to Bedside efforts in reinforcing Case Management skills, and knowledge and attitude per protocol for all disciplines
• Collaborate with site leadership to support staff education efforts
• Facilitates IDG meetings and provides ongoing IDG Support such as triaging incoming calls, provides caller support and assistance, and performs quality calls
• Participates in or lead quality improvement activities, including data collection and aggregation as able
• Reinforces and supports all internal and external communications
