Providence at Home with Compassus is seeking a Hospice Registered Nurse (RN) to provide person-centered care in patients’ homes. You will assess patient and family needs, deliver professional nursing care, and collaborate with the Interdisciplinary Group (IDG) to develop, implement, and direct individualized care plans.

You will conduct comprehensive assessments, write plans of care, reevaluate needs, and participate in on-call duties.

Qualifikationen

• Required: Associate or Bachelor’s degree from an accredited School of Nursing.
• Preferred: 1 year of experience practicing as a registered nurse.
• Current, unencumbered RN license.
• National Provider BLS – American Heart Association (Vendor Managed) within 30 days of hire.

Aufgaben

• Completes assessment of patients and families to determine Hospice needs.
• Provides complete physical assessment and history and professional nursing care using the nursing process.
• Writes and initiates the Plan of Care in collaboration with the IDG.
• Regularly reevaluates patient, family, and caregiver needs and revises the Plan of Care.
• Initiates the Plan of Care and makes revisions as patient status changes.
• Develops a care plan with patient, family, caregiver, and IDG based on nursing diagnosis and palliative actions.
• May administer medications and treatments as prescribed by the IDG Plan of Care.
• Participates in on-call duties as defined by on-call procedures.
• Maintains productivity standards as set by guidelines.
• Completes, maintains, and submits accurate clinical notes.
