Job summary

Providence at Home with Compassus is seeking a compassionate Hospice Registered Nurse (RN) to provide person-centered care in patients’ homes. You will assess patient needs, deliver professional nursing care, and coordinate with the IDG to develop and direct individualized care plans.

Responsibilities include performing comprehensive assessments, initiating and revising care plans, administering prescribed treatments, and participating in on‑call duties while maintaining productivity standards

Qualifications

• Required: Associate or Bachelor’s degree from an accredited School of Nursing.
• Preferred: 1 year of experience practicing as a registered nurse. Experience as a Hospice CNA or Hospice LPN will be considered.

Responsibilities

• Completes assessment of patients and families to determine Hospice needs.
• Provides a complete physical assessment and history and provides professional nursing care by utilizing all elements of the nursing process.
• Writes and initiates the Plan of Care in collaboration with the IDG.
• Regularly reevaluates patient, family, and caregiver needs and revises the Plan of Care as necessary.
• Initiates the Plan of Care and makes necessary revisions as patient status and needs change.
• Develops a care plan in conjunction with the patient, family, caregiver, and IDG that establishes goals based on nursing diagnosis and incorporates palliative nursing actions.
• May administer medications and treatments as prescribed by the IDG‑approved Plan of Care.
• Participates in on‑call duties as defined by the on‑call procedures.
• Maintains productivity standards as set by organizational guidelines.
