Providence at Home with Compassus in Washington state is seeking a Hospice Registered Nurse to deliver person-centered care in patients’ homes. You will assess needs, provide professional nursing care through the nursing process, and collaborate with the IDG to plan and direct individualized care.

The role requires a current RN license, strong assessment and documentation skills, and the ability to work with multidisciplinary teams in end‑of‑life care.

Competenze

• Required: Associate or Bachelor’s degree from an accredited School of Nursing.
• Preferred: 1 year of experience practicing as a registered nurse. Experience as a Hospice CNA or Hospice LPN will be considered.

Mansioni

• Completes assessment of patients and families to determine Hospice needs.
• Provides a complete physical assessment and history and provides professional nursing care by utilizing all elements of the nursing process.
• Writes and initiates the Plan of Care in collaboration with the IDG.
• Regularly reevaluates patient, family, and caregiver needs and revises the Plan of Care as necessary.
• Initiates the Plan of Care and makes necessary revisions as patient status and needs change.
• Develops a care plan in conjunction with the patient, family, caregiver, and IDG that establishes goals based on nursing diagnosis and incorporates palliative nursing actions.
• May administer medications and treatments as prescribed by the IDG‑approved Plan of Care.
• Participates in on‑call duties as defined by the on‑call procedures.
• Maintains productivity standards as set by organizational guidelines.
• Completes, maintains, and submits accurate and relevant clinical notes regarding the patient’s condition and care provided.
• Supervises ancillary personnel and delegates responsibilities when required.
• Participates in the orientation, education, and development of team members.
• Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and in‑service classes.
