Job summary

Pathways Hospice is seeking an RN to join our growing team in the Northwest, Southwest & Southeast Houston service areas. This role focuses on directing home care, coordinating with physicians, and ensuring compassionate patient care. Shifts available PRN and on-call.

You will assess patient needs, develop care plans, educate families, and document care with the HCHB EMR. Texas RN license and hospice experience preferred.

Qualifications

• Valid RN license in Texas.
• Hospice experience (at least 1 year).
• Knowledge of home health regulations and general healthcare compliance.
• Experience with Home Care Home Base (HCHB) EMR (preferred).
• Excellent communication and people skills are essential.
• Ability to work independently and as part of a team.

Responsibilities

• Oversee nursing care team.
• Communicate and collaborate with physicians, therapists, and other healthcare professionals to ensure cohesive patient care.
• Plan, organize, and direct home care services.
• Conduct assessments of patient care needs and develop care plans.
• Educate patients and their families on managing health conditions, medications, and lifestyle changes.
• Prepare timely, complete, and accurate clinical notes.
• Night and Weekends are required per hospice needs.
