Job summary

Pathways Hospice is seeking an RN to join our Houston-based hospice team. This PRN and On Call role oversees nursing care, collaborates with physicians and therapists, plans home care, and develops patient care plans to deliver compassionate, high-quality service.

Candidates must hold a Texas RN license, have hospice experience, understand home health regulations, and be comfortable with HCHB EMR. Night and weekend coverage is required as hospice needs dictate.

Qualifications

Valid RN license in the State of Texas.
Hospice experience (at least 1 year).
Knowledge of home health regulations and general healthcare compliance.
Experience with Home Care Home Base (HCHB) EMR (preferred but not required).
Excellent communication and people skills are essential.
Ability to work independently and as part of a team.

Responsibilities

Oversee nursing care team.
Communicate and collaborate with physicians, therapists, and other healthcare professionals to ensure cohesive and comprehensive patient care.
Plan, organize, and direct home care services.
Conduct assessments of patient care needs and develop care plans.
Educate patients and their families on managing their health conditions, medication regimens, and any necessary lifestyle changes.
Prepare timely, complete, and accurate clinical notes.
Night and Weekends are required per hospice needs of patients
