Pathways Hospice in Houston is hiring a Registered Nurse to join our hospice care team, providing patient-centered nursing in the Northwest, Southwest and Southeast Houston areas.

The RN will oversee nursing care, coordinate with physicians and therapists, develop care plans, educate families, and maintain accurate clinical notes. This role requires strong communication, independence and team collaboration. Shifts available PRN and on-call, with night/weekend coverage as needed.

Formación

• Valid RN license in Texas.
• Hospice experience of 1 year or more.
• Knowledge of home health regulations and compliance.
• Experience with Home Care Home Base (HCHB) EMR preferred.

Responsabilidades

• Oversee nursing care team.
• Collaborate with physicians and therapists to ensure cohesive patient care.
• Plan, organize, and direct home health services.
• Assess patient needs and develop care plans.
• Educate patients and families on conditions and medications.
• Prepare timely clinical notes.
• Night and weekend shifts required.
