New Age Hospice is seeking a PRN Registered Nurse (RN) to support coverage in League City and surrounding areas. The role involves responding to on-call requests, triaging patient needs, and delivering skilled nursing interventions for pain and symptom management.

You will educate patients and families about hospice care, medications, and comfort measures, while documenting all interactions in the EMR and communicating changes with the interdisciplinary team.

Qualificações

• Active RN license in the State of Texas.
• Hospice 1-2yrs experience preferred.
• Strong organizational, assessment, and time-management skills.
• Excellent communication and customer service skills.
• Valid driver’s license and reliable transportation.

Responsabilidades

• Respond promptly to on-call requests, triaging patient needs via phone and in-person visits.
• Assess patient condition and provide skilled nursing interventions for pain and symptom management.
• Educate patients and families on hospice care, medication administration, and comfort measures.
• Document all patient interactions accurately in the EMR per regulatory and agency standards.
• Communicate with interdisciplinary team members regarding changes in inpatient status.
• Coordinate with pharmacies, DME providers, and other resources as needed.
