Job summary

Amedisys Home Health in Rock Hill, SC is seeking a registered nurse for a PRN home health role. You will coordinate and provide patient-centered care in patients' homes, working with care teams to implement plans and ensure safety and quality.

The ideal candidate has RN licensure in SC, at least one year of clinical RN experience, CPR certification, and a valid driver’s license. The position offers flexible scheduling and a comprehensive benefits package.

Qualifications

• One year of clinical experience as an RN.
• Current RN license, state(s) you are assigned to work.
• Minimum six months home health experience.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.
• Able to assess patient status and age-specific needs.
• Familiar with general computer use and online HR/training systems.
• RN Intern program if less than 6 months RN experience.

Responsibilities

• Coordinates and oversees all aspects of patients' home care.
• Collaborates with patient care teams to develop and implement care plans.
• Educates families on patient condition, needs and care.
• Performs head to toe physical assessments.
• Checks vital signs and manages medications.
• Gives injections, IV therapy or tube feedings.
• Administers and maintains catheters.
• Completes documentation timely and accurately.
• Supervises LPNs and HHAs.
• Regularly communicates patient progress to the clinical manager, physician and care team.
• Other duties as assigned.
